Structure and properties
D-mannose is a naturally occurring monosaccharide that belongs to the group of carbohydrates. It is often extracted from maize or birch and is usually available as a white powder with a sweet taste that dissolves easily in water. It has the distinctive property of being barely metabolised. This means that the monosaccharide reaches the bladder almost unchanged and is excreted from there in the urine. It is a medical device and dietary supplement, available both as a powder and in tablet form. As well as in chemists and pharmacies, D-mannose is also available from online shops such as VITALOVE. Anyone wishing to consume a high-quality product should ensure it is produced from birch. This is a purely natural product, as it is made from the bark of wild birch without any additives. Consequently, this form of dietary supplement is purely plant-based and vegan. In most cases, however, it is produced from maize, which is grown on a large scale worldwide and is therefore readily available at a low cost. Unfortunately, it is often contaminated or genetically modified. Consequently, only products made from birch can be considered purely plant-based.
Mode of action
D-mannose usually only exerts its full effect once it reaches the bladder. There, the natural active ingredient binds to the flagella of E. coli bacteria, which are usually the cause of bladder infections. The bacteria are coated by D-mannose and thus neutralised. As a result, the bacteria cannot adhere to the bladder’s mucous membrane and are instead flushed out with the next passage of urine. This effect also applies to E. coli bacteria that are already attached to the mucous membranes. These, too, are coated. However, this only works because the bacteria do not have a fixed position on the mucous membranes of the bladder wall, but move around from time to time. At these moments, the product can act specifically to wrap around the bacteria’s flagella. The bacteria are thus unable to adhere to the bladder wall and are flushed out with the urine. To support this effect, care should be taken to increase fluid intake whilst taking the supplement. This is the only way to ensure that any adverse experiences associated with taking D-mannose are ruled out. Teas, water or juices can all be consumed for this purpose.
Use and dosage
Always follow the manufacturer’s instructions for use and dosage. In most cases, D-mannose should be taken three times a day for the first three days. The dosage is two grams and should be taken at intervals of approximately eight hours. If you wish to use the product over a longer period for supportive or preventative purposes, the daily dose is approximately two grams. You should drink plenty of fluids whilst taking it to promote urine output. It is best to take it on an empty stomach, as this allows for better absorption.

Although D-mannose is a sugar, it has no effect on blood sugar levels. This is because it is not metabolised and is therefore absorbed into the bloodstream unchanged. The sugar is transported via the kidneys to the bladder and is ultimately excreted in the urine. This form of sugar has a very low glycaemic index – meaning it has a minimal effect on blood sugar levels – and is therefore also suitable for people with diabetes. Children can also take it. However, it is important to adjust the dose according to the person’s age and weight. There are also products that have been specially developed for children and adolescents and are tailored to their needs. In general, however, the use of such dietary supplements should be discussed with the relevant paediatrician. Pets such as dogs, cats or horses can also take the product. Here too, the dose should be adjusted to the animal’s weight, although the mode of action remains the same. Administration should, however, be discussed with the vet.
